First Visit

We are devoted to your comfort and value your time. As such we strive to meet your schedule and accommodate you promptly. Your reserved appointment will consist of an exam and consultation explaining your diagnosis, treatment options, and probable number of visits.  We will answer any questions you may have so that you may remain informed and confident in your treatment choices.  In most cases treatment can be done the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide care.

If applicable, please assist us by providing the following information at the time of your first visit:

  • Your referral form.

  • A list of medications and dose you are presently taking, reason for each, and a copy of your most recent INR if you are taking Coumadin. Please also disclose any current or past bisphosphonate use.

  • If you have dental insurance, please bring your card, policy information, and/or completed forms, along with a driver’s license or other form of state identification.

  • Patients under 18 must be accompanied by a parent or legal guardian.

To ensure that we can accurately diagnose your endodontic problem, please do not take analgesic pain medication at least 4 hours prior to your consultation.

Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, CHF, artificial heart valves and joints, etc.) or if you are on any medications (i.e. heart medications, aspirin, anticoagulant therapy, dialysis, history of bisphosphonate use etc.) or require medication prior to dental cleanings (i.e. antibiotics for pre-medication).


Scheduling

We are available by phone, email, or fax and will schedule your appointment as promptly as possible. If you have pain, every attempt will be made to see you that day. We respect your time and do our best to stay on schedule to minimize your waiting. Due to the fact that the practice provides surgical services, various circumstances may lengthen the time allocated for a procedure. Emergency cases can also arise and cause delays. If this is the case we will notify you. We appreciate your understanding and patience.

Missed Appointments

We understand that at times schedule adjustments are necessary and we kindly request 48-hour notice for cancellations to avoid cancellation fee of $50. During COVID-19 we understand that there may be instances where one is not able to give notice within the requested time frame. In such instance, please notify us as early as possible should a schedule change be needed.
